And sometimes you just bump into somebody who is a bit hard of hearing.
I just came from swimming and back at the changing room an Italian guy who trains there tried to chat with the cleaning lady while she mopped the floor. Every time he said something, she asked 'what?' and then the guy repeated it verbatim, and she would understand. At first I thought she needed a second round to cope with his slightly broken Spanish, while I had no problem with it. But when our favorite Italian was gone and I said something to her in --believe me-- absolutely perfect Spanish, the same thing happened!

I do that when I wasn't anticipating that somebody would talk to me, or when I am doing other things I need to concentrate on at the same time. Sometimes I don't register that the other person talked to me, sometimes I am not sure I understood it, especially when there's a lot of background noise. Sometimes I'm pretty sure that what I understood is not what the other person said. And sometimes I just need a little bit extra time to process the information - the times I ask 'what?' and the very same moment finally understand what the other person had said seem to be innumerable.
I'd like to add two things to what Robarb wrote so well, namely:
- does this happen in situations in which you are sure you have the other person's attention? Now, the signals for that are probably different in the other culture, and so should be turn-taking in a conversation. Part of the issue might be that people didn't expect you to talk to them at that particular moment, even when the topic/way of talking about the topic or the grammar/word choice are perfectly ordinary.
If that's the case, talking to a few people regularly should do the trick, as when you get to know the other person and they get to know you you will get used to each other's habits of taking turns and talking about topics, and once you know several people from the same culture you should notice the differences and similarities between them and learn how to emulate what seems to be the similarity between different people from one culture.
- what about people realizing you are nervous and wanting to encourage you? It happens to me all. The. Time. In any language I am learning. I blush, speak with a low voice, hesitate - and people want to encourage me by praising me. (Which doesn't work for me, because I know exactly what is going on and the praise makes me feel like I must show improvement quickly and so I get even more nervous =D)
