I was petrified of speaking German this past summer while I was in Berlin. With my first host family, they could speak to me in German and I'd understand everything, but alot of the time I replied in English, since they could speak it well.** Only when I was forced to speak German with my second host family because they didn't understand me in English (they said I talked pretty fast), did I notice that it wasn't really that bad :-) Despite that, I'm still pretty shy when I come across native speakers :(
** It didn't help when the host father always laughed when he heard me try to say the word "Brötchen"

As for me, I'm more of a "listener" even in my native language, so.. I try not to be too hard on myself, in a group situation I do my best to speak as much as possible, but it's never been something I've done much in any language, at least with strangers!
But of course I do love speaking to my friends! So what I try to do is make friends whom are patient, and perhaps learning my language too, so then we can both make mistakes and it will help me gain confidence! I'm ok with the fact that I will never be a public speaker! I do encourage people to try to speak if you can, but it can be best to start out in a new language with someone patient, who will tell you when you make mistakes, and make an effort to help you :)

Ahh - if you are a good listener you will be everyone's hero! Being able to listen well will win you many friends.

Although I feel very confident about my receptive skills (reading and listening) and writing, sometimes I feel a little bit uncomfortable when speaking to my teachers at university in English (although it is in Greece, it is tradition and almost a rule for students and teachers - even the non native speakers of English - in the Faculty of English studies in Athens to give lectures, take notes, make questions,give responses and generally speak in English). I do not have any problem when talking to my colleagues and friends in more informal register, but some teachers at school may get rather annoyed if one of us uses inappropriate language or chooses to speak in Greek and I need to be extra careful in such cases. Plus I'm more like an introverted person, which makes things rather complicated.
Nevertheless,I feel less shy about speaking than in the first year of my studies, when I got my first language shock (LOL), and I'm very grateful for having been given so many chances there.
