I think I would have been intending to order two drinks, so perhaps if there was a lot of background noise or someone was struggling to understand me, holding up my index finger and middle finger to mean "two". I probably wasn't paying any attention to what I was doing with my thumb, because it wouldn't have occurred to me that anyone would try to count it. This was mainly in German-speaking countries, so there was confusion for the waiter both in trying to discern whether I had said "zwei" or "drei" and then the fact that even if he thought I had said "zwei", in his opinion I was holding up three fingers so I must have meant "drei".

My boyfriend told me he had the same problem when he first moved to France, and kept ending up with two beers when he just held up his index finger. However he was quite happy with that outcome as a French beer is only about half the size of an English beer anyway :)
