I guess children use it (at least my young cousins), but I think that's because they see it on TV. It's pretty much common on cartoons.

Most people use nossa!, putz!, caramba! and regionally eita!, vôte!, etc.

Eu uso, meu filho já aprendeu a usar e minha família ainda usa muito, especialmente na forma "puxa vida!".

I'd say yes, but at least here in São Paulo "Nossa" is more common I guess.

Pode ser usado como ordem ou pedido para puxar alguma coisa. Mas não era esse o sentido da pergunta... :p

As situações de uso de "puxa vida", para mim, sempre foram as mesmas dadas nesse dicionário on-line: "Expressão de espanto, surpresa, indignação, desagrado".
