Came across this fascinating document on the FSI website.

Adapting and Writing Language Lessons

It starts with "What seems to be what in language teaching", and a discussion of various approaches to language instruction. It mentions that some methods go in and out of fashion, and relates some of the earliest approaches. It also discusses such teaching methods as,
