Yes it is possible that it is really a regionally limited phenomenon. Maybe it is also a question of the social surrounding - don't know. I come from Saxony-Anhalt, which isn't so far away, but maybe already far enough for minor differences in expression.

I'd probably use it in the diminuativ form
"Er ist ein dreckiges kleines Würstchen"...in a humorous way
I live in the Rhinland area btw.
