How difficult is it not to get them confused? Along the lines of Portuguese and Spanish
or are they more distinct like Spanish and Latin or German and Danish?

I would say more like Spanish -> Latin or perhaps Jamaican Patois -> Queen's English.
When I was there most Haitians could understand French fairly well, but the native French speakers couldn't make
out a word of Creole!
