This is my first Log I have made. I am 13 and I believe I am at an Intermediate level in Japanese. Up until this point I have simply used books like Assimil Japanese to learn. But today I started my intensive L-R study. I am working through these titles:
Read Real Japanese: Essays (comes with a CD)
Reading Japanese with a Smile (I request the audio from rhinospike.com)
Breaking into Japanese Literature (mp3s online)
In addition I have been reading books (fiction) from my local Japanese market.
I also watch TV, Movies, Listen to music, etc. in Japanese. I study at least 3 hours daily. I hope that I will have reached an advanced level (C1) by the New Year.

I wish you luck on your recent decision to start using the L-R technique. For me this technique has been heaven sent literally. Japanese went from one big stream of sounds to distinct words in a matter of three days. This was with 7 hours of learning of course with breaks in-between to do my own stuff. Who knew you could gain so much from passive learning; by a native simply reading and following along with them. My schedule is similar to yours but I'm still a baby in Japanese so its slightly different. Best of luck
