Easy, I say "Google?" and it says, for instance,

"Aap ka naam kya hai" is more commonly used AFAIK.
If you are already acquainted with someone then you probably already know his/her name. In hindi, degrees of familiarity in ascending order are: Aap<Tum<Tera Someone who's very familiar would be asked "Tera naam...?" Someone who's less familiar or a child would be asked "Tumhara naam...?" If you want to sound more formal, you could ask, "Aap ki tashrif?" I'm not sure though. Forgive me if I digress but I have a question for native urdu speakers. When adressing a man you say "Janab" and then ask his name right. How do you address women? How do you address a child?
