Don't say to a Japanese person you have 心の問題(kokoro no mondai which is lit. 'heart problem') ... I tried to say I have 'heart problems' but was promptly told a few minutes later, "That means you suffer from mental problems." ^^;
edit: This was back when literally translating things was a good idea.
