I was taught writing dej, mej in the late seventies and I still do. As the the spelling is close the spoken language it reads better for me. A similar thing is seen when writing loan words. I find it interesting that most people would write tejp and not tape, but fewer write mejl rather than mail. I prefer e-post though, as I'm now officially middle-aged!

I can't remember having seen it in any book we used in school, nor being suggested by any of our teachers. I started school as a seven year old in August 1980.

One reason might be that I'm from Gotland were nobody (with a fairly standard Gotlandic accent) even pronounced the pronouns as mej, dej, sej (but rather mi, di, si).

I write 'tejp', 'mejl', 'jos' and 'skajpa', though I, too, prefer 'e-post'.

In response to the first question.
The others have already answered the question, but I want to add we do sometimes follow the written phonetics when we want to emphasize the word in question: 'ja vill inte ha kaffe' (no emphasis) vs. 'Jag vill inte ha kaffe' (emphasis on 'jag').
In southern Sweden we pronounce 'mig' and 'dig' like 'mai' and 'dai' resp. (a sound similar to the 'i' in the English word 'fight')

Much depends on the dialect and the word in question. In some dialects, for example in southern Sweden, consonants are not strongly pronounced and words tend to float together more (abit like French vs. Italian), with the effect that vowels clearly dominate the spoken language.
Even simple words like 'är' (att vara = to be), is pronounced quite differently, in southern Sweden we just say 'e': Hur e de ? (written: hur är det ?).
Other dialects say it "correctly" (i.e. follow the written style). It is not a problem which dialect you learn, everyone understands eachother anyway.

One strange phenomenon I see becoming more and more common among younsters is to mistakenly write 'dem' instead of 'de'. For example, some people write 'dem är inte hemma', which is equivalent to writing 'them are not at home'. I suppose they think it is the written form of 'dom', not realizing that 'dom' corresponds to both 'de' and 'dem'.

Written form, maybe... I haven't thought of that. They must have seen "de", right?

I've always seen it as (just) an example of hypercorrection. The Swedish kids (and a lot of adults...) happen to misinterpret the usage of "dem" and then use it wrongly (since they must never write "dom").

I would say that we rather pronounce the words "jag", "mig", "dig", "det" and "de" in the following way:
jag = ['ja]
mig = ['mej]
dig = ['dej]
det = ['de]
de = ['dom]
