There are a number of potential problems with language exchange and I think that, as mentioned by a few others already, it takes a lot of tinkering before you find what (who) works for you. Those of us living abroad or otherwise in our target environment get a few more shots at figuring it out than the rest. Unfortunately, many probably aren't that lucky.
I think a few things contribute to the success of my current language exchange:
1) Similar levels - Both of us are capable of carrying on a conversation entirely in our L2
2) Similar motivation - We both use our L2 for business and must continue to improve
3) Equal time - We decide the amount of time per partner before starting.
4) Similar interests - We met because of a mutual interest in martial arts and through this and languages actually became friends. Going to each others' martial arts demonstrations, attending parties and events together, and occasionally just getting sloshed all contribute.
