I haven't seen that yet in horizontal writing, but in vertical it's very common to have dots (they look like commas) to the right of emphasized characters. Since right is the "upstream" side of vertical text, it makes sense that dots would go above in horizontal writing.
That means that I'd expect underlining to go on the left side of vertical text, but I haven't seen that yet either.
