I really like these two series and I think they could serve you well as they have been helping me greatly. Each comes in three volumes and leads from the beginning to the advanced stages, one to B2, one to the C levels
Gramática de uso del Español
by Ediciones SM
three levels, A1-A2, B1-B2, C1-C2
Gramática
by Anaya E.L.E. En
three levels: A1-A2, B1, B2
all are in Spanish only, with exemples and exercises (with key of course)
Another thing you might find very useful is a verb book, which can be found online as well (sorry I cannot remember where to find the best one). To get all the model, irregular and other verb tables for reference.

I think this might be what you're looking for:
http://www.amazon.com/Exacto-Practical-Guide-Spanish-Grammar /dp/0071396519/ref=sr_1_2?
ie=UTF8&qid=1408607904&sr=8-2&keywords=exacto+spanish#custom erReviews
It's both a reference grammar and a workbook that is useful for beginners through high-
intermediates. The exercises and explanations for each grammatical point are divided into
groups according to level.

De nada! As far as I know, there is only 1 volume. There's a 2nd edition, which
means something quite different: that the original text has been revised and/or updated
in some way. Sometimes these revisions or updates are significant, but more often they're
very minor and just provide a way for the publisher to make more money; people are less
likely to buy used when they see that a "new" edition has been released.
I've never seen the 2nd edition of Exacto, so can't say if it's any better or worse than
the 1st. But odds are it's close to identical, besides the cover.
