Is the 'th' sound used in most of Spain or just some places? i.e CerbeTHa instead of CerbeSa.

I believe it's used pretty much everywhere in Spain except Andalucía.

Not strictly mainland Spain but in the Canary Islands, like Andalucía, seseo is also the norm.
EDIT: Should also add that in parts of Andalucía - especially round Cádiz and some rural areas as well - you will also find ceceo (pronounced THeTHeo).
So not only will cerveza be pronounced as cerveTHa but casa will be pronounced as caTHa.

Not entirely true. As Andy pointed out, it occurs in the Cádiz province in Andalucía, especially in rural areas.

I'm confused! so is it widely used in Spain or not? Madrid, Barcelona? for someone hoping to learn Spanish used in Spain itself, Is it worth an outsider using the 'th' sound or not

Yes, it is widely used in Spain and certainly in Madrid. I think people will understand you equally well whether you use the "th" or "s" sound. So I guess it's up to your personal preferences.
