Actually, I call my parents Mutti and Vati, and I know many people who do the same. I also hear Mama/Mami and Papa/Papi a lot, but I have to say, for me that sounds very childish if it's an adult. On the other side, Mutter/Vater sound very formal or old-fashioned. So I suppose, Mutti/Vati is some kind of compromise - neiter too childish nor too formal.
I don't know anyone who addresses his parents by their first name, though, and I'm fairly certain that at least my parents wouldn't like that.

Odd, even in my extended family (one half has a lower middle class background/NRW, the other one is of rural background/Hessen) people normally use the names to address each other, making exceptions only for young children and for outsiders who don't know the first name yet. If I find some time this week end I'll try to see if I can find something on the topic.
