So far, I have done:
For German:
-three years of high school courses
-fragmented self-studying
For Spanish:
-read through the "Everything Learning Spanish" book and took a lot of notes
-spent a solid month trying to figure out how to do an alveolar trill, though I still can't put it into words very well and the single 'r' gives me a LOT of trouble when paired with d's and t's
-Pimsleur I twice (though doing 2 or 3 lessons a day both times) and now on lesson 11 of Pimsleur II
-Michel Thomas Foundation through Advanced CD 3 (I will start CD 4 tomorrow)
-Rosetta Stone Spanish Level I and half of Level II (my computer got a virus and it hasn't been fixed yet, but I think I will drop the RS anyway)
For Russian:
-some university studies and e-mails with Russian friends
For French:
-nothing :P
EDIT: I have decided to modify this into a "TAC" log.
I guess I'll rate my current standing (as of the 25th of November, 2008) in my intended languages of study:
German and Spanish: mid-upper beginner
Russian and French: nil!

Today I finished the final CD of MT Advanced and did lesson 12 of Pimsleur twice. I think I'm going to go through MT Advanced one more time to try to get the verb tenses embedded in my memory a bit better and see if I can find any of the other MT courses at the library.
EDIT:
I forgot to mention that I decided to start doing Mnemosyne SRS stuff to see if that helps me out any. Over the course of a few sessions I went through all 750 entries in one of the Spanish sets I downloaded off the Mnemosyne website to put them into rotation. A lot of them were familiar to me and even more were cognates.
I do have a question about SRS, right now I have each entry entered as Spanish Question/English Answer and English Question/Spanish Answer, so each Spanish word is in there twice, once as the question and once as the answer. Is this a good way to go about it or is this wasting more time than it is worth?

-found a book at the library called "A Frequency Dictionary of Spanish" so I wrote down the words up to #500 that I didn't already know (like el/la/a/de and some common verbs/nouns that all beginning Spanish courses use pretty extensively). I typed the first 70 or so into Mnemosyne (giving me about 970 entries, or about 485 words/phrases since each is in there twice).

24 Nov.:
-finished MT Advanced -- 1 hr
-Pimsleur lesson 16 twice -- 1 hr
-picked up the MT Builder from the library, but I'm not really sure if it is going to be very effective for me. Did half of CD 1 -- approx. 40 minutes
-SRS reviews, there were about 180 to do and I was able to recall about 90% of them, although I am still not sure what the most effective way to do them is. I've read mixed reviews about going from Target language to Native language and vice versa, currently I am using both.

25 Nov.:
-finished CD1 of the MT Builder -- approx 40 minutes
Towards the end I think I was dozing off and responding automatically instead of actually thinking about what I was saying. I'm going to try to just finish CD2 in one sitting tomorrow, it is pretty boring though.
-Pimsleur lesson 17 twice -- 1 hr
By contrast, this lesson seemed to go by really quickly.
-SRS reviews -- about 400
I added the rest of the words on the list I took from the frequency dictionary, maybe 246 words. I should head back tomorrow and write down some more because I really feel like this is having quick tangible effects on my reading comprehension. Oh, and I decided that I am just going to do Spanish->English cards from now on.
After I finish all the Michel Thomas things, I think I may start on the FSI courses. I may decide to finish Pimsleur level II first, but I do want to jump into FSI while all the grammar from MT is still really fresh (I have a feeling that MT covers more grammar than Pimsleur).

I believe it's fine. I've never really done it en masse with just one way.
But, I can suspect that doing it one way will increase only active or only passive and not both.

You don't have to input your words twice. There is a checkbox "Add vice versa too" in the window for adding cards. The window for editing the deck has this option too when you right click entries.

And tja, that's what I've been doing (for adding new cards) but I didn't know about the option when editing the deck, which might be really convenient if I change my mind again later on, so thanks!
And today I finished the MT Builder course. I ended up enjoying the second CD more than the first for some reason, I guess because it was less of a review and more of a "builder". All in all, I'm glad I went through the whole thing. I also did Pimsleur lesson 18 once, I would've liked to have done it again, but I've just finished the builder course now and it's pretty late. Also did my SRS reviews for the day and was a little disappointed that I didn't retain a lot of the words I put in yesterday. Oh well, I did add a lot of words and I'm pretty sure that after a few days they'll start to fall in line.
So today:
MT Builder -- approx 80 minutes
Pimsleur -- 30 minutes
