All of them are wrong. You are using the VTT, or
voltooid tegenwoordige tijd, which requires the
voltooid deelwoord, not the verleden tijd (woonde), so it has to end with "gewoond".
"Want" gives a reason, so that sentence makes no sense, but if you are asking the
period of time that the person started to live there, I would just use simply, "Sinds
wanneer heb je daar gewoond?". It can also use "er" instead if both interlocutors know
where the location is, as "daar" has slightly more emphasis.
If the person still lives there in the present, "Sinds wanneer woon je daar?" sounds
better.

Whoops! The issue was the first part of the sentence and I forget to get the "gewoond"
right.
So let me try again with this version:
Hoe lang heb je daar gewoond?
Want hoe lang heb je daar gewoond?
Voor hoe lang heb je daar gewoond?
Sinds hoe lang heb je daar gewoond?
According to the Internet usage, all these forms are frequently used (Hoe lang, Want hoe
lang, Voor hoe lang, Sinds hoe lang) with "Want hoe lang" the least frequent. "Want" in
this case doesn't mean "because" but "since", so I do think it makes sense. The following
sentence was asked by the Dutch moderator of 'Blauw Bloed" in June. "Want hoe lang
woont u al in Canada?" during the recent visit of Willem-Alexander and Maxima.

This is the only form in the first search that I found so far where I find "want hoe
lang"
Want hoe lang duurt het nu echt voor je conditie weer bij af is?
"sinds hoe lang", "voor hoe lang", or "sinds wanneer" just based on what I would
choose automatically is what comes to my mind if talking about a certain duration of
time with start and end and/or has relevance in the present.

Want hoe lang = because how long exactly? It implies that they're looking for a specific
answer.
1e4e6's statement is something like "So how long does it REALLY take before your
condition is back to zero?"

I would use none of the above. As a student of Dutch I would ask this:
Hoe lang woon je al in Belgiƫ? (How long have you been in Belgium?)
If it's all over and in the past:
Hoe lang heb je in Belgiƫ gewoond? (How long were you in Belgium?)
